Please note the following before completion
of the form:
- Applications are required to
be submitted with endorsement by The AFC HENLEY
Membership Secretary at the bottom of the form.
- Eligibility. The applicant
must a) live in the town of Henley or the parishes of Bix, Rotherfield Greys or Remenham;
or b) be attending a
publicly maintained school or college in one of those areas; or c) have attended a local
school or college for at least two
years.
- Applications
can also be made for sports footwear and prescription sports eyewear.
